I went to Dr Elgey and had 4 mls of dermal filler which he strategically placed in the temples, cheeks and chin and I am honestly blown away by the results. It’s a real game changer! Cheek filler aftercare: Dr Paul's tips for the best results

Proper aftercare is essential to the best results and a comfortable recovery. Here are the guidelines to help you care for your skin after cheek filler.

Avoid alcohol and blood thinners

Alcohol and blood-thinning medication such as aspirin and ibuprofen increase the likelihood of bruising and swelling. It is best to avoid these for 24 to 48 hours after treatment to let your body heal.

Staying hydrated and avoiding alcohol for a couple of days will help your body recover faster and minimise any swelling or bruising. It is a small adjustment that can make a big difference.

Avoid pressure or massage

Pressure on the treated area can cause the filler to shift or settle unevenly. For the first 24 to 48 hours, avoid massaging or applying pressure. Gentle cleansing is fine, but avoid any direct manipulation of the filler.

Be mindful of how much pressure you apply to your face in the first couple of days, and avoid heavy make-up or skincare that requires rubbing. This keeps the filler in place and achieves the contour we planned.

Monitor for signs of infection

Some swelling, bruising or redness is normal, but persistent or worsening symptoms such as excessive pain, warmth or discolouration should be taken seriously. If you notice any sign of infection or a vascular complication, contact the clinic immediately.

I always stress that if something feels wrong, it is better to call. Early signs of a complication can be subtle, but addressing them quickly can prevent more serious issues.

Avoid heat exposure

Avoid high-temperature environments such as saunas, steam rooms and strenuous activity for at least 48 hours. Heat increases blood flow to the treated area, which can intensify swelling and bruising and may shorten how long the filler lasts.

I advise steering clear of any heat-based activity for at least 24 hours, including very hot showers or baths. It is all about letting the filler settle without risking more swelling.

Keep the treated area clean

Keeping the area clean helps avoid infection. Wipe surfaces like phones and glasses regularly in the first 24 hours, and avoid touching the treated area while the injection sites heal.

I remind patients to use a clean pillowcase after treatment and wipe their phone regularly. It is so easy to touch your face without realising, and these small steps significantly reduce the chance of infection.

Use cold compresses to reduce swelling

A cold compress, not ice, applied intermittently during the first 24 hours can help minimise swelling and bruising. Apply it gently and never directly on the skin, to avoid irritation.

Cold compresses work wonders for swelling, but apply them gently. Wrap the compress in a soft cloth so you do not shock the skin with direct cold.

Stay hydrated

Hyaluronic acid filler attracts water, so drinking plenty of fluids helps the filler settle and supports your skin. Hydration also aids recovery and helps minimise swelling.

Elevate your head while sleeping

Sleeping with your head elevated stops fluid building up in the treated area and helps reduce swelling, especially in the first few nights. Try not to lie directly on the treated area for the first 24 to 48 hours so the filler can settle.

Sleeping with an extra pillow or two is a simple way to reduce swelling overnight. It is comfortable and effective, and it helps the treated area heal properly.

Gentle skincare and sun protection

Avoid harsh skincare and active ingredients such as retinol for at least a week after treatment. Sun protection matters too: use a broad-spectrum sunscreen of SPF 30 or higher to prevent UV damage, which can degrade the filler.

After any filler I recommend a gentle, hydrating skincare routine for a week, and always a good SPF, even on cloudy days. Sun protection keeps your skin safe and helps prolong your result.

How long do midface fillers last?

The longevity of midface filler results varies depending on the type of filler used and individual metabolic factors. On average, hyaluronic acid fillers last between 12 to 18 months, but can be longer depending on individual factors. Factors such as the amount of filler injected, the area treated, and the individual’s lifestyle can influence the duration of the results.

Are there any risks?

While midface filler treatments are generally safe, they do carry some risks. Swelling and bruising can occasionally be seen but typically resolve within a few days. Infection is rare, but possible if post-treatment care instructions are not followed. Lumps and bumps can occur but are often temporary and can be massaged out or dissolved if persistent. Asymmetry is rare but possible, and usually correctable with additional filler.

How much filler will I need?

This is assessed at consultation and depends on how much midface support has been lost, your facial proportions, and the result you are after. Cheek filler is usually built conservatively, and a staged approach across sessions is often preferred to overfilling in one appointment.

Is there downtime after cheek filler?
