Just had a second polynucleotides treatment under the care of Dr Paul. He took time to explain the treatment, downtime and recovery periods. All of this was done within a friendly engaging environment where he encouraged questions. I'm super happy with the results and the professionalism of how Dr Paul consulted me, especially as I had initially wanted tear trough injections but after review advised on a different course, of polynucleotides. Certainly will be back, and can't recommend enough.

I had a great experience with Paul for lip filler and Sunekos injections. He is attentive, friendly, and very knowledgeable. He took the time to understand my facial structure, how it moved to ensure a nice balanced and natural look. Also appreciate that he isn’t afraid to say no to more treatments so that things don’t look overdone. Will always be happy to recommend!

I went to Dr Elgey and had 4 mls of dermal filler which he strategically placed in the temples, cheeks and chin and I am honestly blown away by the results. It’s a real game changer! I went in feeling super nervous but Dr Elgey is so knowledgeable, professional and warm - I quickly felt at ease. We spoke at length about my concerns and then he advised me on where the filler should be placed to get the best results. I had absolutely no bruising afterwards and swelling was minimal. The result is so natural but I look so much younger and fresher. I honestly wish I had done it sooner.

Paul is fantastic. He uses his experience to understand what you’re looking for and advise on what may work, he does not push or upsell any treatments and he has a real creativity and specialises in achieving a natural result. I trust his knowledge and skills, and his ethics.

I had a consultation and UltraClear treatment with Dr Paul for acne scarring with other skin concerns and my experience has been excellent from start to finish. After speaking with several practitioners, what stood out most was Dr Paul’s honesty and knowledge. I never felt pressured into treatment, and he took the time to explain the reasoning behind his recommendations. Throughout the process, he has been incredibly responsive and supportive, answering all of my questions before and after treatment. He also prepared a personalised pre-treatment skincare cream which made me feel very well looked after. The recovery has also gone smoothly thanks to the clear aftercare instructions and Dr Paul’s ongoing support.

Highly recommend Paul. He really put me at ease, showed careful consideration and thought for the fact I was coming to him after having previous filler in my lips dissolved due to migration, and kept me very well informed throughout the process. Clearly highly skilled and I am so happy to have found someone I can trust with my lips for the long term!

The treatment

What is tear trough filler?

Tear trough filler is a non-surgical treatment designed to reduce the appearance of dark circles, hollows and bags under the eyes. A small amount of hyaluronic acid is placed in the under-eye area to create a smoother, more youthful appearance, ideal for anyone looking to refresh the eye area without surgery.

Dermal filler can also be placed in the upper eyelid to improve the hollowing and dark shadows that come from volume loss, known as A-frame deformity. Because the under-eye connects to the cheek, Dr Elgey assesses the midface at the same time, as restoring cheek support often improves the tear trough too.

Indications and applications

What Can Tear Trough Filler Treat?

Tear trough filler is particularly effective for:

Soften dark circles

Restores light to the under-eye area, reducing the shadowing that makes the eyes look tired.

Restore under-eye volume

Adds subtle fullness to hollow under-eye areas, reducing the depth of the groove between the lower lid and the cheek.

Refresh a more youthful look

Smooths fine lines and softens the signs of ageing around the eyes, including hollowing in the upper lid, for a rested appearance.

Refine the result to suit you

Every treatment is tailored to your features and goals and dosed conservatively, so the outcome stays natural.

Fit treatment around your life

A quick procedure with results visible immediately and minimal downtime, so you return to your day the same day.

Treatment science

How Does Tear Trough Filler Work?

Tear trough fillers work by adding volume to the under-eye area, reducing the depth of the tear troughs and smoothing out hollows.

Hyaluronic acid, a naturally occurring substance in the body, is injected in the under-eye area creating a hydrating and plumping effect in the skin, diminishing dark circles and creating a more refreshed look.

The under-eye is thin, mobile and unforgiving, so placement is deliberately conservative. A small volume in the right plane does more here than a larger volume ever could.

Understand the Science Behind Treatment

What can be treated?

A conservative amount of filler softens the hollows and shadows that make the eyes look tired, always assessed alongside the midface.

Technique and safety

How your tear trough filler is placed

The tear trough is one of the more advanced areas to treat, so filler is placed deep, close to the bone in conservative volumes with a blunt cannula, and assessed alongside the midface, for a refreshed, natural result rather than an overfilled one.

Dr Paul Elgey, aesthetic doctor, London
  • Placed deep and close to the bone to support the under-eye and soften the shadow
  • A blunt cannula and small volumes, staged rather than overfilled in one session
  • The filler contains lidocaine, so the area numbs as the gel is placed
  • The midface is assessed at the same time, as cheek support often improves the tear trough
  • Fully reversible with hyaluronidase if a result is ever not right

Tear Trough Filler Treatment Protocol

The treatment protocol for tear trough fillers at my London clinic typically starts with an initial consultation to discuss your goals and assess your suitability for the procedure.

On the day of the treatment the filler is injected using a cannula. The procedure usually takes about 30 minutes, and the results are immediate. Post-treatment care includes avoiding excessive facial movements and refraining from applying pressure to the treated area for at least 24 hours.

Pricing

Tear Trough Filler Pricing

Pricing depends on:

  • Whether the under-eye alone or the upper lid is treated too
  • The volume needed to correct the hollow
  • Whether the midface needs supporting first

Not everyone with dark circles is a candidate for tear trough filler. If your shadowing is pigment rather than hollowing, I will tell you at consultation and suggest something more appropriate. You are quoted once the plan is agreed.

Why Choose Dr Paul Elgey?

The tear trough is the highest-risk area on the face for filler, and the one where poor work shows most. It needs anatomical knowledge, a cannula, and the judgement to decline.

With over 20 years of medical practice and advanced aesthetic training, my focus is:

  • Safety
  • Precision
  • Natural outcomes
  • Long-term skin health

A good under-eye result is one nobody notices. It should read as rest, not as treatment.

Tear Trough Filler FAQs

If you are curious about the procedure, aftercare, or potential side effects, the answers to common questions are below. The under-eye is the most technique-sensitive area on the face, so every plan is assessed individually.

Are there any risks?

While tear trough filler treatments are generally safe, they do carry some risks, including: Swelling and Bruising: This can happen occasionally but typically resolves within a few days. Infection: Rare, but possible, if post-treatment care instructions are not followed. Lumps and Bumps: Can occur but are often temporary and can be massaged out or dissolved if persistent. Asymmetry: Rare but possible, and usually correctable with additional filler.

How long does tear through filler last?

The longevity of tear trough filler results varies depending on the type of filler used and individual metabolic factors. On average, hyaluronic acid fillers in this area last between 18 to 24 months, but often longer. Factors such as the amount of filler injected, the area treated, and the individual’s lifestyle can influence the duration of the results. We will guide you on how to maintain your results.

What types of tear trough filler are used?

Several types of fillers can be used for the tear trough area. Each has its own properties and the choice of filler depends on your individual needs. We can advise you on the various options to ensure the best results.

What techniques are used for tear trough filler?

Various techniques can be employed to achieve different enhancements in the under-eye area: Linear Threading: Involves injecting the filler in a linear fashion to create volume and smoothness. Microdroplet Technique: Involves injecting small amounts of filler for precise contouring and natural-looking results. Fan Technique: A method used to distribute filler evenly and achieve a natural contour. Each technique is chosen based on the patient’s unique under-eye anatomy and desired outcome, ensuring a customised and aesthetically pleasing result.
